What's your scope of practice and how often do you prescribe this medicine?
If you're not available, who do I contact?

About the medicine

Which specific medicine (brand and active ingredient) will I be prescribed?
Is this within the UK licensed indication, or off-label?
If off-label, what's your clinical reasoning and how is it documented?
What's the typical dose schedule and titration plan?
Which pharmacy will dispense it?

About me

Given my medical history, is there anything that makes this less appropriate?
Should I have baseline blood tests before starting? Which ones?
Are there interactions with my other medicines or supplements?
If I'm of reproductive age — what's the contraception plan and washout for pregnancy?

About risks

What are the common side effects?
What are the rare but serious side effects?
What symptoms mean I should stop the medicine and contact you?
What symptoms mean I should call 111 or 999?

About expectations

What outcome can I realistically expect, and over what timescale?
How will we know if it's working?
What's the plan if it isn't working at 12–16 weeks?
How long can I stay on this?

About logistics

How often will we meet for follow-up?
How do I escalate side effects between appointments?
What does this cost monthly, including consultations and the medicine?
What's the policy if I want to stop?

Symptoms to mention

Nausea or vomiting
Constipation
Diarrhoea
Abdominal pain (location? radiating?)
Reflux / heartburn
Injection-site reaction
Headache or visual changes
Fatigue or low mood
Hypoglycaemia signs (sweating, shaking, confusion)
Right-upper-quadrant pain
Severe upper-abdominal pain radiating to the back
Allergic-type reaction (swelling, hives, breathing difficulty)

History of pancreatitis
History of gallbladder disease
History of thyroid conditions (especially MTC or MEN-2)
Mental-health history (depression, anxiety, eating disorders)

Renal or hepatic conditions
Alcohol intake
Smoking
Other healthcare providers I see (specialist, dentist, etc.)

Red flags / commitments

I will not buy unlicensed peptides online or via social media DMs.
I will not buy ‘compounded’ GLP-1s from clinics that won't name the prescriber and pharmacy.
I will not double-dose or push the titration faster than agreed.
I will report adverse events on Yellow Card.
I will stop the medicine and contact you if I develop severe abdominal pain, severe vomiting, signs of pancreatitis, or signs of severe allergic reaction.
